The essential calculators every medical student should know: Anion Gap and ABG Interpreter (Internal Medicine), Wells PE + PERC Rule (Emergency Medicine), CURB-65 (Pneumonia), SOFA + qSOFA (Sepsis), GCS (Neurology/ICU), eGFR (Nephrology), ASCVD Risk (Cardiology/Primary Care), PHQ-9 + GAD-7 (Psychiatry/Primary Care), MELD + Child-Pugh (Hepatology), and BMI (every rotation). Mastering the clinical reasoning behind each score matters as much as the number itself.

Elevated anion gap (>12 mEq/L) = excess unmeasured anions. Use the MUDPILES mnemonic: Methanol, Uremia, DKA, Propylene glycol, Isoniazid/Iron, Lactic acidosis, Ethylene glycol, Salicylates. Next steps: (1) Check delta-delta ratio ([AG − 12] ÷ [24 − HCO₃]) — if >2, suggests concurrent metabolic alkalosis; if <1, suggests concurrent non-AG acidosis. (2) Correct AG for hypoalbuminemia: add 2.5 mEq/L per 1 g/dL drop in albumin below 4. (3) Order lactate, BMP, urine ketones, and toxicology screen based on clinical context.

Five-step ABG interpretation: (1) pH — acidemia <7.35, alkalemia >7.45. (2) Primary process — low pH + low HCO₃ = metabolic acidosis; low pH + high PaCO₂ = respiratory acidosis; high pH + high HCO₃ = metabolic alkalosis; high pH + low PaCO₂ = respiratory alkalosis. (3) Compensation — use Winter's formula for metabolic acidosis (expected PaCO₂ = 1.5 × HCO₃ + 8 ± 2); check if compensation is appropriate. (4) Oxygenation — PaO₂ and A-a gradient. (5) Clinical correlation — always interpret with the full clinical picture.

Start with pre-test probability: if clinical suspicion is low, apply PERC first — if all 8 criteria are negative, PE is excluded without D-dimer. If any PERC criterion is positive, or pre-test probability is intermediate, calculate Wells PE. Wells 0–1 (low): D-dimer; if negative, PE excluded. Wells 2–6 (intermediate): D-dimer; if positive, proceed to CT-PA. Wells >6 (high): proceed directly to CT-PA without D-dimer. The PERC rule only applies in low pre-test probability settings — never use it when you already think PE is likely.

CURB-65 scores: Confusion (new), Urea >7 mmol/L (BUN >19 mg/dL), Respiratory rate ≥30, Blood pressure (SBP <90 or DBP ≤60), age ≥65. Score 0–1 = low severity (outpatient treatment); 2 = moderate severity (short admission or close outpatient follow-up); ≥3 = high severity (hospitalization, consider ICU for score 4–5). For immunocompromised patients, pregnant patients, or those with poor social support, clinical judgment should override the score — CURB-65 is a decision support tool, not a rule.

Report eGFR using CKD-EPI 2021 (without race variable). Present CKD stage alongside: Stage 1 = eGFR ≥90 (with kidney damage markers); Stage 2 = 60–89; Stage 3a = 45–59; Stage 3b = 30–44; Stage 4 = 15–29; Stage 5 = <15 (dialysis). Always include trend (acute vs chronic decline), proteinuria (urine ACR), and medication implications. For drug dosing, use Cockcroft-Gault CrCl not eGFR. Highlight nephrotoxic medications and renally-cleared drugs requiring dose adjustment.
