The National Early Warning Score 2 (NEWS2) aggregates six physiological parameters — respiratory rate, SpO₂, supplemental oxygen, systolic BP, heart rate, level of consciousness (ACVPU), and temperature — into a 0–20 score. Score 0–4 = low risk (routine monitoring); 5–6 or any single parameter scoring 3 = medium risk (urgent review by clinician); ≥7 = high risk (emergency assessment, consider ICU). NEWS2 is the standard early warning tool in NHS and many international hospital systems for detecting clinical deterioration.

The Richmond Agitation-Sedation Scale (RASS) is assessed at every nursing check in mechanically ventilated patients: +4 = combative, +3 = very agitated, +2 = agitated, +1 = restless, 0 = alert and calm, −1 = drowsy, −2 = light sedation, −3 = moderate sedation, −4 = deep sedation, −5 = unarousable. Target RASS −1 to 0 (light sedation) per PADIS guidelines reduces ICU complications. A RASS of −3 or better is required before delirium assessment with CAM-ICU is reliable.

The Critical-Care Pain Observation Tool (CPOT) scores four behaviors: facial expression (0–2), body movements (0–2), muscle tension (0–2), and compliance with the ventilator or vocalization (0–2), total 0–8. A score ≥3 indicates significant pain warranting analgesia. CPOT is validated for adult ICU patients who cannot self-report pain and is recommended by PADIS guidelines alongside the BPS. Reassess 15–30 minutes after an analgesic intervention to confirm adequate pain control.

MUST (Malnutrition Universal Screening Tool) should be completed on all patients within 24 hours of hospital admission and weekly thereafter, or when clinical condition changes. Score 0 = low risk (routine care); 1 = medium risk (document food and fluid intake for 3 days, review if adequate); ≥2 = high risk (refer to dietitian, initiate nutrition support plan). Malnutrition is present in up to 40% of hospitalized patients and significantly increases length of stay, infection risk, and mortality.

The insulin correction factor (sensitivity factor) estimates how much 1 unit of rapid-acting insulin lowers blood glucose. It is calculated using the '1800 rule': 1800 ÷ total daily insulin dose (TDD). For example, TDD = 50 units gives a correction factor of 36 mg/dL per unit. Correction dose = (current glucose − target glucose) ÷ correction factor. Always verify the patient-specific correction factor from their diabetes care plan or prescriber, and document pre- and post-correction glucose levels per protocol.

Mean arterial pressure (MAP = [SBP + 2×DBP] ÷ 3) reflects organ perfusion pressure. The standard MAP target is ≥65 mmHg for most critically ill patients, including those in septic shock (Surviving Sepsis Campaign guideline). Higher targets (≥75–80 mmHg) may be prescribed for patients with chronic hypertension, TBI (cerebral perfusion pressure = MAP − ICP), or renal impairment. Sustained MAP <65 mmHg (>3–5 minutes) should trigger immediate nursing escalation and vasopressor review.
