Mini-ACE (M-ACE) is a concise 30-point cognitive test derived from the Addenbrooke's Cognitive Examination, designed for rapid outpatient and bedside cognitive triage. It samples multiple cognitive domains and is used to identify patients requiring fuller neurocognitive assessment.

How It Works

Complete Five Scored Tasks

The examiner administers five tasks: (1) immediate recall of a 7-word address phrase, (2) verbal fluency (animals named in 60 seconds), (3) clock drawing, (4) delayed recall of the address (3 points), and (5) abbreviated cognitive items. Total score 0-30.

Score and Apply the Threshold

Sum item scores. A score of 25 or below is the most widely used threshold for possible cognitive impairment, with reported sensitivity of 88% and specificity of 89% for dementia in published validation studies. Administration takes approximately 5 minutes.

A positive Mini-ACE screen (score <=25) should prompt extended assessment—full ACE-III, MoCA, or formal neuropsychological evaluation—plus collateral history, functional review, and appropriate specialist referral.

Verbal Fluency Is Highly Informative

The animal naming fluency item (60 seconds) is one of the most sensitive single items for early dementia. Normal performance is typically 15-18 animals per minute in cognitively intact older adults; fewer than 12 strongly suggests impairment.

Clock Drawing Captures Multiple Domains

The clock drawing subtest reflects executive function, visuospatial ability, and semantic memory simultaneously. Use a standardized scoring criteria—command (draw clock showing 11:10) with a pre-drawn circle provided—and document the specific error types observed.

Delayed Address Recall Is Most Sensitive

The 3-point delayed address recall item is often the single most discriminating item for dementia. Even one uncued recall failure in a patient with concerns warrants follow-up. Offer category cues before scoring failures.

Mini-ACE was validated primarily in English-speaking populations with moderate educational attainment. Apply caution with patients having less than 8 years of formal education or non-native English speakers—borderline scores may reflect language exposure rather than pathology.

Mini-ACE screens for impairment but cannot characterize dementia subtype, severity stage, or functional impact. Always integrate with clinical history from the patient and an informant before making management decisions.

Test at a time of day when the patient is most alert. Late afternoon testing in older adults may underestimate cognitive reserve. Reassure anxious patients that the test has no pass or fail—this reduces performance anxiety effects.

Compare to Full ACE-III When Borderline

When Mini-ACE scores fall in the 25-27 range with clinical concern, proceed to the full ACE-III which provides 100-point detailed domain scoring across memory, language, attention, fluency, and visuospatial domains.
