The Pfeffer Functional Activities Questionnaire (FAQ) is an informant-rated measure of instrumental activities of daily living. It covers complex tasks such as finances, shopping, meal preparation, remembering appointments, and transportation. Total scores range from 0 to 30, with higher values indicating greater functional impairment.
Formula: FAQ total = sum of 10 informant-rated items (0-30).

Higher FAQ totals indicate greater instrumental-function impairment and support higher-level care and safety planning.
