How It Works
Rate 4 Items About the Past 2 Weeks
The patient rates 4 items on a 0-3 scale (Not at all / Several days / More than half the days / Nearly every day) about the past 2 weeks. The first 2 items are the GAD-2 anxiety items; the last 2 items are the PHQ-2 depression items.
Calculate Total Score and Two Subscale Scores
Anxiety subscale (GAD-2): sum items 1+2, range 0-6. Depression subscale (PHQ-2): sum items 3+4, range 0-6. Total PHQ-4 score: sum all 4 items, range 0-12. No reverse scoring needed.
Apply Subscale and Total Thresholds
Anxiety subscale: score 3+ = positive anxiety screen (GAD). Depression subscale: score 3+ = positive depression screen (PHQ). Positive PHQ-4 Subscale Should Trigger Full PHQ-9 or GAD-7
PHQ-4 is a triage instrument. A PHQ-2 subscale score of 3+ should trigger full PHQ-9 administration (9 items). A GAD-2 subscale score of 3+ should trigger full GAD-7 administration (7 items). Both can be triggered simultaneously if both subscales are positive. PHQ-4 is not designed to replace these more comprehensive assessments.
PHQ-4 Subscale Scores Are More Clinically Actionable Than Total Score
The PHQ-4 total distress score (0-12) is useful for global distress monitoring, but the individual subscale scores (depression PHQ-2 and anxiety GAD-2, each 0-6) are more clinically actionable. Kroenke et al. (2009) showed that PHQ-4 total scores of 6+ (moderate-severe distress range) correlate with significant functional impairment, disability days, healthcare utilization, and reduced quality of life. Patients scoring 6+ warrant immediate full assessment regardless of subscale pattern.
PHQ-2 Depression Items Capture the Two Core DSM Criteria
The PHQ-2 depression subscale captures the two core DSM-5 major depressive episode criteria: (1) depressed mood and (2) anhedonia (loss of interest or pleasure). These two items are sufficient to generate clinically relevant depression screening because the presence of either or both at threshold severity defines the diagnostic gateway criteria for major depression.
GAD-2 Anxiety Items Are the Two Highest-Loading GAD-7 Items
The GAD-2 items (feeling nervous/anxious/on edge, inability to stop or control worrying) are the two items with the highest factor loadings in GAD-7 and the two items most strongly associated with GAD diagnosis. Despite being only 2 items, GAD-2 achieves sensitivity 86% and specificity 83% for generalized anxiety disorder.
