RA Treatment Overview
Conventional synthetic DMARDs — methotrexate first-line, with sulfasalazine, hydroxychloroquine, and leflunomide as alternatives or add-ons — remain the backbone of initial RA therapy. Renal involvement affects up to 50-60% of lupus patients at some point and is one of the leading drivers of lupus-related morbidity and mortality. SLEDAI-2K's renal descriptors (proteinuria, hematuria, pyuria, urinary casts) provide an early warning signal, but formal lupus nephritis assessment requires a quantitative urine protein-to-creatinine ratio, careful renal function trending with eGFR, and — when active nephritis is suspected — a kidney biopsy to grade the histologic class and guide immunosuppression choice (typically mycophenolate mofetil, cyclophosphamide, or belimumab/voclosporin as add-on therapy per current guidelines).

Hydroxychloroquine is now considered foundational therapy for essentially every lupus patient regardless of severity — it reduces flare frequency, protects against organ damage accrual, and even shows a modest survival benefit, with an excellent long-term safety profile aside from the well-known (and rare at approved doses) retinal toxicity requiring periodic ophthalmologic screening. Acute Management vs. Long-Term Urate-Lowering Therapy
These are two distinct treatment tracks that are easy to conflate. Acute flares are treated with NSAIDs, colchicine, or corticosteroids (oral or intra-articular), chosen based on renal function, GI risk, and comorbidities — not with urate-lowering drugs, which do nothing for an active flare and can even prolong it if started mid-attack without anti-inflammatory cover.
Long-term urate-lowering therapy (allopurinol first-line, febuxostat as an alternative) is indicated for patients with recurrent flares (generally ≥2 per year), tophi, urate nephrolithiasis, or evidence of joint damage. The treat-to-target goal is serum urate below 6 mg/dL (below 5 mg/dL if tophi are present) — mirroring the same treat-to-target philosophy used in RA with DAS28, just with a lab value instead of a composite clinical score. When to See a Rheumatologist
Primary care and urgent care can reasonably initiate workup for suspected inflammatory arthritis, but referral to rheumatology is appropriate whenever: joint swelling persists beyond a few weeks without a clear alternative explanation, serology (RF, ACPA, ANA, anti-dsDNA) comes back positive alongside compatible symptoms, a patient has multi-system symptoms suggestive of lupus (rash plus joint pain plus unexplained cytopenias, for example), or gout flares are recurring frequently enough to warrant long-term urate-lowering therapy planning. Earlier referral generally means earlier treatment — and for all three of these conditions, that window matters more than almost anything else in determining long-term outcomes.

What's the difference between RA and osteoarthritis? RA is an autoimmune, inflammatory condition causing symmetric joint swelling, prolonged morning stiffness (often >1 hour), and systemic inflammation (elevated CRP/ESR, positive RF/ACPA in most cases). Do normal lab results rule out these conditions? Not necessarily. RA can be seronegative (normal RF/ACPA) in a meaningful minority of patients. ESR and CRP can be normal in active RA, particularly in older patients or those with lower muscle mass. Serum urate can be normal during an acute gout flare. In all three conditions, the clinical picture and, where available, direct evidence (synovial fluid crystals for gout, biopsy for lupus nephritis) carry as much or more weight than any single lab value.
