The 2015 ACR/EULAR Gout Classification Criteria apply only to patients who meet the entry criterion: at least one episode of swelling, pain, or tenderness in a peripheral joint or bursa. Without this, the criteria don't apply at all. Once met, check the sufficient criterion first: if monosodium urate (MSU) crystals are identified on polarized light microscopy of synovial fluid from a symptomatic joint, bursa, or tophus, this alone is enough to classify the patient as having gout — no further scoring across the clinical, laboratory, or imaging domains is needed. MSU crystal identification remains the diagnostic gold standard because it's directly visualizing the causative pathology (negatively birefringent, needle-shaped crystals), rather than inferring the diagnosis from an indirect clinical and laboratory pattern. If synovial fluid aspiration isn't feasible or crystals aren't found, proceed to the points-based scoring in the next step.
When crystal confirmation isn't available or is negative, score across eight items in three categories. Clinical domain: joint/bursa pattern (MTP1 involvement scores 2, ankle/midfoot without MTP1 scores 1), episode characteristics (erythema over the joint, can't bear touch or pressure, and significant difficulty walking or using the joint — 1 point per characteristic present, up to 3), time course (recurrent typical episodes — rapid onset within 24 hours, resolution within 14 days, complete symptom resolution between episodes — score 2 for recurrent, 1 for a single typical episode), and clinical tophus (a draining or chalk-like subcutaneous nodule under transparent skin, often over extensor surfaces, elbows, or ears — scores 4 if present). Laboratory domain: serum urate measured by a uricase-based method, ideally when the patient is not on urate-lowering therapy and at least 4 weeks from the start of the flare (urate can transiently drop during an acute attack) — scoring ranges from -4 points for urate under 4 mg/dL (makes gout unlikely) up to +4 points for urate ≥10 mg/dL. Synovial fluid analysis showing MSU-negative crystals (if fluid was analyzed but no crystals found) subtracts 2 points.
Imaging domain contributes two possible findings, each worth 4 points if present: evidence of urate deposition on ultrasound (the double-contour sign — an irregular hyperechoic band over the articular cartilage surface) or dual-energy CT (DECT, which can directly visualize urate deposits color-coded distinctly from calcium), and evidence of gout-related joint damage on conventional radiography (at least one erosion in the hands or feet). Neither imaging modality is required to apply the criteria — they're optional but can push a borderline score over threshold. Sum all domain points (clinical + laboratory + imaging, with the synovial fluid MSU-negative penalty subtracted if applicable) for a total possible range of -6 to 23. A total score of 8 or more classifies the patient as having gout. Scores below 8 don't rule out gout — especially if urate testing, imaging, or synovial fluid analysis wasn't performed — but indicate the available evidence doesn't yet meet the classification threshold.

Apply these criteria alongside a septic arthritis risk assessment for any acutely swollen, painful joint — a high gout classification score with classic MTP1 (podagra) involvement and typical rapid-onset time course supports gout, but joint aspiration to rule out infection remains essential whenever septic arthritis can't be confidently excluded, since the two can occasionally coexist.
Primary care physicians and internists
When arthrocentesis isn't readily available or feasible (e.g., a small, hard-to-aspirate joint), the points-based clinical, laboratory, and imaging criteria offer a validated alternative path to classification using history, exam, serum urate, and potentially ultrasound imaging alone.
Rheumatologists and primary care managing recurrent gout
A confirmed gout classification, combined with recurrent flares, tophi, or urate nephrolithiasis, supports initiating long-term urate-lowering therapy (allopurinol, febuxostat) per ACR gout management guidelines, which recommend treat-to-target serum urate below 6 mg/dL (or below 5 mg/dL with tophi).

Scoring the specific joint pattern (MTP1 strongly favors gout, while knee and wrist involvement is more typical of calcium pyrophosphate deposition disease) alongside serum urate and imaging helps differentiate gout from pseudogout when synovial fluid crystal analysis is inconclusive or unavailable.

Modern gout trials use the 2015 ACR/EULAR criteria for standardized enrollment, ensuring a consistent, validated diagnostic threshold across study sites rather than relying on inconsistent clinical impression alone.
If synovial fluid analysis is feasible and shows MSU crystals, you're done — this is diagnostic on its own regardless of serum urate level (which can be misleadingly normal or even low during an acute flare) or any other clinical feature. Don't let a normal serum urate during an acute attack override a positive crystal analysis.
Urate levels transiently drop during acute gout attacks in a meaningful proportion of patients, likely from urate's acute-phase uricosuric effects and cytokine-mediated renal excretion changes. A single normal urate during a flare doesn't rule out gout — for the most accurate classification-criteria scoring, retest when the patient is asymptomatic and at least 4 weeks from flare onset, ideally off urate-lowering therapy.
First metatarsophalangeal joint involvement is present in about 50-70% of first gout attacks and up to 90% of patients at some point in their disease course — its high specificity is reflected in its 2-point weighting versus 1 point for ankle/midfoot involvement without MTP1.
For a patient scoring 5-7 points from clinical and lab findings alone (just under the 8-point threshold), a point-of-care ultrasound showing the double-contour sign adds 4 points and can push classification over threshold without needing joint aspiration, particularly useful for small or difficult-to-aspirate joints.
Chronic tophaceous gout deposits aren't always the classic large, draining nodule — early or small tophi can appear as firm, slightly yellow-white subcutaneous nodules, sometimes visible through thin skin over the ear helix or finger extensor tendons, and are easy to miss on a cursory exam.
If fluid was analyzed and no MSU crystals were found, this scores -2 points rather than 0, reflecting that a truly negative tap in a symptomatic joint meaningfully argues against gout (though sampling error and inexperienced crystal reading can produce false negatives, so clinical correlation still matters).
Gout and septic arthritis can occasionally coexist, and a high gout classification score should never substitute for synovial fluid Gram stain and culture in a joint where infection is a genuine concern (fever, marked leukocytosis, immunocompromise, or an atypically severe presentation) — always send fluid for both crystal analysis and culture when infection can't be confidently excluded.
MSU crystal identification alone is sufficient for gout classification. Otherwise, sum points across clinical, laboratory, and imaging domains — a total of 8 or more out of 23 classifies gout. Scores below 8 don't rule out gout, particularly if urate testing, synovial fluid analysis, or imaging wasn't performed.
Use these criteria when evaluating a patient with at least one episode of peripheral joint or bursal swelling, pain, or tenderness to formally classify gout, particularly when synovial fluid crystal analysis is unavailable, not feasible, or was negative despite a suggestive clinical picture.
The criteria classify gout but don't exclude coexisting septic arthritis, which should always be considered separately when infection is a clinical concern. Serum urate can be misleadingly normal during an acute flare, and imaging findings require ultrasound or DECT expertise not universally available.
