The 2010 ACR/EULAR Rheumatoid Arthritis Classification Criteria replaced the older 1987 ACR criteria to identify RA earlier in its course, before the joint erosions and nodules the 1987 criteria required. Four weighted domains — joint involvement, serology, acute-phase reactants, and symptom duration — sum to a score out of 10, with ≥6 classifying definite RA in a patient who has synovitis in at least one joint not better explained by another disease. Once RA is classified, track ongoing disease activity with DAS28. If serology is negative and the clinical picture is atypical, consider SLEDAI for lupus-associated arthritis or Anion Gap and eGFR as part of a broader autoimmune/systemic workup.

Before applying the 2010 ACR/EULAR criteria at all, the patient must have clinical synovitis (swelling) in at least one joint that isn't better explained by another disease — degenerative joint disease, gout, psoriatic arthritis, systemic lupus erythematosus, or another inflammatory arthropathy. This is a prerequisite gate, not a scored domain: a patient without objective synovitis, or whose synovitis is clearly explained by an alternative diagnosis, should not be scored with these criteria regardless of serology or symptom duration. This is a critical distinction from purely symptom-based tools — the criteria are designed for classification (largely for research and epidemiologic consistency) in patients who already have undifferentiated inflammatory arthritis, not as a general screening test for anyone with joint pain.
Joint involvement is scored based on the number and size of affected joints, examined for swelling or tenderness consistent with synovitis: 1 large joint scores 0, 2-10 large joints (shoulders, elbows, hips, knees, ankles) scores 1, 1-3 small joints (with or without large joint involvement) scores 2, 4-10 small joints scores 3, and more than 10 joints including at least 1 small joint scores 5. Small joints include MCPs, PIPs, second through fifth MTPs, thumb interphalangeal joints, and wrists; distal interphalangeal joints, first carpometacarpal joints, and first MTP joints are excluded as they're commonly affected by osteoarthritis. Serology is scored using rheumatoid factor (RF) and/or anti-citrullinated protein antibody (ACPA, commonly reported as anti-CCP): negative for both scores 0, low-positive for either (≤3× the upper limit of normal) scores 2, and high-positive for either (>3× ULN) scores 3. A patient needs only one of RF or ACPA positive at the higher tier to get the full 3 points — both being positive doesn't add further points beyond using the higher of the two results.
Acute-phase reactants scores 0 if both CRP and ESR are normal, and 1 if either is abnormal — only one point is available regardless of how many markers are elevated. Symptom duration scores 0 for less than 6 weeks and 1 for 6 weeks or longer, based on patient-reported duration of signs or symptoms (pain, swelling, tenderness) of joints currently clinically involved, regardless of treatment status. Sum all four domains for a total score from 0 to 10. A score of 6 or higher classifies the patient as having definite RA, provided the entry criterion (synovitis not better explained by another disease) is met. Critically, a score below 6 does NOT rule out RA — the criteria are designed with high specificity for classification purposes, and patients scoring below 6 who continue to have unexplained inflammatory synovitis should be reassessed over time as duration accrues past 6 weeks or as serology/acute-phase markers evolve, since early RA can present with a score below threshold that increases at follow-up.

Walking a patient through exactly which criteria they met — joint count, positive serology, elevated inflammatory markers, duration — makes an abstract diagnosis concrete and helps patients understand why early, consistent DMARD therapy is being recommended.
These criteria were designed for classification with an emphasis on capturing patients early, but a patient with 3-4 weeks of symptoms and only mild serologic abnormality may simply not have accrued enough duration or serologic clarity yet. Re-score at follow-up as the 6-week duration threshold is crossed or repeat serology returns, rather than dismissing RA based on one early, borderline assessment.
Osteoarthritis, crystal arthropathy (gout, pseudogout), psoriatic arthritis, and viral arthritis can all mimic early RA's joint pattern. Skipping this exclusion step and jumping straight to scoring the four domains is the most common misapplication of these criteria and can lead to misclassification.
If RF is low-positive (2 points) and ACPA is high-positive (3 points), the serology domain scores 3, not 5. Double-positive serology (both RF and ACPA positive) does carry prognostic significance for more aggressive disease and erosive risk, but it doesn't add extra classification points beyond the higher single result.
A patient with 15 large joints involved but no small joints scores lower on the joint domain (capped effectively without ever reaching the 5-point tier, since that tier requires at least 1 small joint) than a patient with just 4 small joints. This reflects the higher specificity of small joint (particularly MCP/PIP/wrist) synovitis for RA versus large joint disease alone.
In patients with early undifferentiated arthritis and negative RF, checking ACPA specifically (rather than assuming negative RF means negative serology overall) can meaningfully change the classification score and often predates clinical synovitis by months to years in patients who go on to develop RA.
These joints are commonly affected by osteoarthritis, and including them would reduce the specificity of the joint domain for inflammatory RA-pattern disease. Don't count DIP swelling toward the small joint tally even if it appears inflamed — consider psoriatic arthritis instead, which characteristically involves DIP joints.
A patient can be clinically diagnosed and treated for RA by an experienced rheumatologist based on overall gestalt even with a score just under 6, particularly with strong ACPA positivity and classic small-joint symmetric polyarthritis — the criteria standardize research and epidemiologic classification but shouldn't override sound clinical judgment in ambiguous individual cases.
A total score of 6 or higher out of 10, in a patient with clinical synovitis in at least one joint not better explained by another disease, classifies definite rheumatoid arthritis. Scores below 6 don't exclude RA but indicate the patient doesn't yet meet formal classification — reassessment over time is often warranted.

The criteria are designed for classification and research consistency rather than as a universal diagnostic gate — clinical judgment can support an RA diagnosis and treatment even with a borderline score. They also require synovitis not better explained by another disease as a prerequisite, so they cannot be applied to patients without objective joint swelling.
